We are looking for an enthusiastic, motivated pharmacist to join our team. You will gain valuable clinical knowledge and experience across all specialties and be supported through a rotational clinical training programme. You will also be encouraged to study for a PG Diploma in Clinical Pharmacy.

Someone with excellent communication and organisational skills you will have a desire to thrive and learn in a clinically focused team.

Main duties of the job

Following a structured induction programme you will join our team providing clinical pharmacy services, as part of an operational rotation in order to facilitate the provision of safe, effective and cost-effective pharmaceutical care.

Rotations include general medicine, general and specialist surgery, orthopaedics, elderly care, emergency admissions, and stroke.

Provision of 24/7 access to pharmacy services including evening, weekend and bank holiday working to provide;

  • Clinical pharmacy advice
  • Medicines information
  • Dispensing and supply services

To undertake a structured development and post-graduate training programme.

Job description

Job responsibilities

Clinical Practitioner

  • To provide clinical pharmacy services, as part of an operational rotation in order to facilitate the provision of safe, effective and cost-effective pharmaceutical care this includes:
  • Assessing patients on admission and referring any identified as requiring level 2 or level 3 pharmaceutical care to the appropriate Senior Clinical Pharmacist, Lead Pharmacist or Consultant Pharmacist.
  • Personal delivery of level 1 clinical pharmacy, minimising the clinical risk associated with medicines use by ensuring that:
  • prescriptions are clinically coherent, legally acceptable and complete,
  • doses are acceptable and there are no clinically significant interactions or allergies apparent
  • Formulary arrangements and/or local guidelines are being complied with
  • medicines are supplied at the appropriate time,
  • medicines are administered in accordance with prescriptions.
  • Delivering elements of level 2 clinical pharmacy as planned and delegated by the responsible Clinical Pharmacy Team Leader, Specialist Pharmacist or Consultant Pharmacist.
  • Provision of evidence based advice given at individual patient care levels to:
  • Medical, nursing, pharmacy and other health care professionals both within the trust and the wider health community

Provision of Out-of-hours Pharmaceutical Services

  • To provide out-of-hours pharmacy services, covering all patients of the Trust and external customers. The service is based at Doncaster Royal Infirmary but provides cover for all Trust sites and those of external customers. This includes acting as duty pharmacist during evenings and weekends.
  • To participate in the first on-call rota
  • Clinically verifying prescriptions received for dispensing during the out-of-hours/on-call periods, in order to minimise the clinical risks associated with medicines use.
  • Provision of accuracy checks for items dispensed by technical staff and during on-call periods personally undertaking dispensing and self-checking.
  • Personal delivery of urgent medicines information and advice when requested by medical staff and senior nursing staff.
  • Provision of professional leadership and clinical supervision to the out-of-hours team of technical staff.
  • Ordering urgent supplies of medicines required out-of-hours from suppliers and other hospital pharmacy departments.
  • To respond to urgent requests for medicines supplies from other hospital pharmacy departments out-of-hours.
  • To respond to, and action, urgent (class 1) medicines recalls received out-of-hours

Undergo Training

  • Following an induction programme foundation pharmacists join a structured training programme which is integrated with their operational rotation. The training programme is also designed to support study towards a postgraduate diploma in Clinical Pharmacy.

Other Duties

  • To achieve objectives agreed at Performance Review with Line-Manager in order to effectively contribute to the systematic provision of services within the Pharmacy Clinical Services Section, ensuring progressive innovation.
  • To participate in the audit, clinical trials and practice research activities of the Directorate at an appropriate level.
  • To participate in the weekend, Bank Holiday, and first on-call rotas, in order to ensure that patients receive timely and appropriate pharmacy services
