Job summary
We have excellent opportunities to join the established Community Therapy Team at Doncaster Royal Infirmary. You will be involved in the care of patients requiring therapy assessment in their own homes. You will work as part of a therapy team and with the wider multi-disciplinary team (MDT). This may include inter-organisational working, providing effective rehabilitation and signposting to services to allow patients to live as independently as able at home.
You will be required to independently manage your own caseload of patients including complex patient management as well as supporting other members of the team. You must have excellent communication and time management skills. Documentation is an integral part of the role, computer skills are essential as you will be recording patient information on the Clinical Therapy electronic systems.
Excellent interpersonal skills and the ability to motivate patients throughout their journey is essential as part of this role.

Main duties of the job
You will provide specialist assessment, diagnosis and treatment of patients with complex problems as an autonomous practitioner with the Community Physiotherapy Team.
You will support the Team Lead in the operational management of the team and in service developments on a local or service-wide level
You must have excellent communication and interpersonal skills, with the ability to motivate not only patients but staff. You must be organised, flexible and able to empower others.
You will provide therapy assessment and treatment to patients without our community and will work closely with our partner organisations to provide timely input for our patients.

- To provide highly specialist physiotherapy assessments over a range of conditions and complexities using advanced clinical reasoning to analyse clinical and non-clinical findings to form accurate physiotherapy diagnoses
- To assess patient understanding of assessment and treatment in accordance with the Consent Policy, and to assist members of the team to work within the legal framework for patients who lack capacity to consent to treatment
- To provide health education, health promotion and prevention advice
- To manage a clinical caseload working autonomously, but with access to advice/support as required
- To formulate and to ensure delivery of an evidence-based, clinically effective treatment programme, to meet the agreed goals set with the individual patient and/or carer
