Specialist Community Dietitian

Job summary
We are looking for a dietitian to join our primary care team and can offer opportunities in community and mental health inpatient areas, outpatient clinics and specialist weight management. You will be part of our primary care dietetic team but will have support from a range of clinical specialist dietitians from across Northamptonshire. We are keen to work with you to develop your knowledge and skills and can offer you a competency package to complete if your feel you are not ready for a band 6 post yet but are interested in moving to Northamptonshire. We are looking for a dietitian who is an excellent communicator, a team player, has a patient centred approach to care and keen to develop their clinical practice.
Main duties of the job
The role involves:
o Providing dietetic assessment, advice, treatment, monitoring and counselling to support behavioural change in patients who meet the referral criteria. This will be delivered through one-to-one interviews, group sessions and community-based clinics. Some of this work is now done virtually
o Autonomously managing a complex caseload and work with patients and their carers to agree patient centred care plans
o Working collaboratively with the wider multi- disciplinary health care team to ensure delivery of coordinated care and to provide specialist advice and attend case conferences, network meetings as appropriate.
o Evaluating patients' progress and collecting outcomes
o Contributing to and implementing service improvement work
o Using a range of verbal and non-verbal communication tools to communicate effectively with patients to implement treatment plans.
o Providing advice and training to patients, carers, colleagues and other healthcare professionals to promote an understanding of the aims of dietetic treatment to ensure a consistent approach to patient care.
o Managing clinical risk within own caseload
o Inputting into the training of dietetic students and other health care professionals
