Job summary

Northamptonshire Healthcare NHS Foundation Trusts, community physical health team are looking for a qualified Band 5 General Nurse (RGN) wo can inspire, promote, and deliver comprehensive physical health screening for our mental health patients. This is a busy role where you will able to deliver your nuring skills and expertise to make a valubale difference to our community patients.

You will work within a multidisciplinary Physical Health Team comprising healthcare assistants, administrators, bank staff, and a health coach, all reporting to the Physical Healthcare Lead (Band 7 RGN). You will collaborate closely with CMHT staff, UCAT, senior management, Clinical Systems, Pharmacy, and in-patient teams across the organisation. You'll also liaise with external partners including GPs and local acute hospitals (Northampton General Hospital and Kettering General Hospital).

After completing induction, training, and probation, the successful applicant will support the Physical Healthcare Lead by overseeing the team and its workload in their absence, making managerial and clinical decisions as needed, and providing senior guidance to junior colleagues. They will work closely with multidisciplinary team (MDT) members to coordinate timely appointments and ensure the efficient delivery of physical healthcare services, while maintaining regular communication with the Physical Healthcare Lead.

Main duties of the job

The main duties are as follows but not limited to:

  • Screen mental health patients for underlying physical health issues and monitor for side effects of treatment.
  • Assess, plan, develop, implement and evaluate programmes to promote health and wellbeing, and prevent adverse effects on health and wellbeing of our patient group.
  • Regular monitoring and interventions for patients with long term conditions such as diabetes, asthma, hypertension, epilepsy etc
  • Perform clinical procedures including. Electrocardiograms (ECG) & Blood taking (Phlebotomy)
  • Act as a point of contact for advice and support to junior staff in the absence of the team leader.
  • Conduct regular audits to ensure high standards of care and compliance
  • Travel to patients' homes and clinic sites across the county - a full UK driving licence and access to a vehicle is essential
  • Use Microsoft 365 confidently for documentation, communication, and reporting.
