Job summary
The post holder will work as part of the specialist Community Younger People with Dementia team, offering support to clinical staff, service users and their carers. Visits are mostly offered in service users homes or localities across Northamptonshire, therefore car driver and own transport is essential.
The post holder will be expected to manage an autonomous caseload, offering interventions as agreed by the Care Co-ordinator.
The post holder will also be required to undertake Physical Health Checks (PHC's) (e.g. Blood pressure and weight and pulse) Feed back to Nurse Prescriber within the team.
Ability to work with service users with a progressive neurological condition- supporting them to live well in the community with dementia this will involve working closely with the Occupational Therapist.
Main duties of the job
Support service users to live well with Dementia and remain as independent as possible.
Support the Occupational Therapist - with collating information re groups and offering support in the groups as required.
To enable the service user to access community resources, which would include accompanying service users to groups until settled- Planned approach regularly reviewed.
- To support and educate the service user and their families and carers as to local and national resources available.
- To support and help completing forms for benefits as these are working age people and the diagnosis would affect all aspects of functioning.
- Communication is an integral part of the role needed to work closely with service users and their families.
- Need to be computer literate able to complete accurate records on the clinical notes. Creating easy read resources and communicating with service users about group events planned by the team.
