Job summary

Healthcare Assistants working within the Community Intermediate Care team have a rewarding and varied job role. They visit patients in their own homes and play a fundamental part in improving the lives of people when they are at their most vulnerable.

We are looking for enthusiastic applicants with experience of working in a health and social care background and multi-disciplinary working. The post holder will provide assessment and rehabilitation to patients in their own homes and ensure patients are referred on for longer term needs.

Main duties of the job

Main duties of the job

As a Healthcare assistant you will:

  • Support the team to avoid patients being admitted to hospital by providing short term rehabilitation and support at home
  • Provide rehabilitation for patients who have become deconditioned being in hospital to ensure they are discharged from hospital in a timely manner
  • Assess and provide suitable equipment to enable people to live independently
  • Be accountable for providing quality care to our patients
  • Utilise technology to enable you to update records as you go

Patients treated by this team are often complex and require a multidisciplinary response to treat and rehabilitate them in their own environment. With a fantastic ethos our dedicated team has a passion for quality care.

Must have skills

You will need to have some experience of working in a healthcare setting, ideally based in the community. You will need to adapt to different scenarios in the community and be able to work with a range of health professionals and informal carers. You will need to be able to drive and have access to a car for work purposes to enable you to fulfil your community role.

You will be part of a dedicated team who have a passion for quality care and should be able to work flexibly with minimal supervision.
