Job summary
The post holder will work as part of the specialist Community Young People with Dementia team, offering support to clinical staff, service users and their carers. The post holder will be expected to work autonomously and hold a caseload relevant to their role, offering interventions as agreed by the Care Co-ordinator. The post holder will be required to work closely assisting service users and carers with applying and accessing benefits, liaising with social care and local resources available along with undertaking Physical Health Checks (PHC's) (e.g. Blood pressure) required prior to be commenced on medication.The post holder will require some background knowledge and understanding of dementia and desirably mental health.The post of the Support Worker will need to apply a strength-based approach to be able to work collaboratively with service users, carers and care co-ordinators to facilitate the following:They will contribute to assessments, planning, implementation, and evaluation of care provided to service users.They will work with the Multi-disciplinary Team which includes Nurses, Occupational Therapist, and Psychologists. The care co-ordinator will have overall responsibility for the care provided with each service user, with you being the identified support worker for a service user in the team when needed will also be another point of contact for them and their carer.
The teams work across Northamptonshire with clinics we can access if required countywide.
Main duties of the job

1. To work under direction of clinical staff, to ensure individuals are appropriately supported and facilitate links with internal and external specialist services as required2. To be actively involved with service users, carers and care co-ordinators in the planning and delivery of services (towards their identified goals)3. To support in partnership with clinical staff, service users and their carers in the planning, implementation and reviewing of care4. To support the service user to gain access to benefits and welfare rights assisting in this when required.5. Escorting service users to day centres/other various visits as determined by the care co-ordinator and as part of the individuals plan of care. 6. Take part in and encourage service users to participate in occupational and community recreational activities7. To do pieces of individual, planned work with services users under direction of clinical staff8. Help to identify early signs of deterioration and to relay these observations back to clinical staff in a timely fashion.9. Provide support with the wider team to service users in crisis10. To liaise closely with the full range of services within the local community, statutory and non-statutory services
