Job summary
Due to maternity leave in the department, we have an opportunity to provide cover for 2 days (0.4wte) Band 7 community work. You will work with 3 Practices in Northampton: Bugbrooke Surgery, Kings Heath Practice and Park Avenue Medical Centre. You will work closely with the primary care team and will have enhanced clinical skills to enable you to manage a varied adult caseload but will focus on weight management , nutritional support and diabetes
Main duties of the job
o Providing dietetic assessment, advice, treatment and counselling to support behavioural change in patients who meet the referral criteria. This will be delivered through one-to-one interviews, group sessions and community-based clinics in a face to face environment.
o Autonomously managing a complex caseload and work with patients and their carers to agree patient centred care using evidence based interventions
o Working collaboratively with the wider Practice and primary and secondary care healthcare team to ensure delivery of coordinated care and to provide specialist advice and attend case conferences, network meetings as appropriate.
o Evaluating patients' progress and collecting outcomes
oMake recommendations to PCN and Practice stff regarding changes to medications for the nutritional management of patients based on interpretation of biochemical, physiological and dietary requirements and ensure prescription requests are cost effective.
oContribute to and implement service improvement work
o Using a range of verbal and non-verbal communication tools to communicate effectively with patients to implement treatment plans and to use behavioural change skills in your daily practice.
o Providing advice and training to patients, carers, colleagues and other healthcare professionals to promote an understanding of the aims of dietetic treatment to ensure a consistent approach to patient care.
