About us
Within Northamptonshire we are continuing to transform our core community and crisis pathway services to meet the needs of our local population and meet the ambitions of the NHS Long Term Plan. Through co-production, we are reviewing and developing multi-agency and integrated pathways that are responsive to people's needs; we are developing place-based teams to enable people to receive care closer to home. Whilst we are redesigning pathways, we are developing an ethos and culture that transforms how we work together as one team sharing care so that service users and carers experience high quality care which is needs led and seamless.
NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
