Job summary
A fantastic opportunity has arisen for an experienced Nurse Practitioner or Paramedic to take on a senior role in this fast-paced, thriving and challenging community based Intermediate Care Team which has been delivering healthcare at home in Northamptonshire for 25 years. We pride ourselves on our passion for delivering quality care to patients in their own homes, avoiding unnecessary hospital admissions and facilitating rapid discharge from hospital to enable patients to return to independent living by working in collaboration with all community services and Social Care providers.
Our Intermediate Care team are innovative in their clinical practice, continuously developing new patient pathways with partner organisations and using remote monitoring equipment to manage a Virtual Ward. Their focus on admission avoidance and patient flow has been instrumental in ensuring patients can remain at home whenever possible. Recently to major focus has been on increasing our rapid response capability and opening up pathways with the ambulance Trust and on developing our virtual ward. The clinical team includes both nurses, doctors and therapy staff.
We are seeking a dynamic, flexible, and experienced Nurse Practitioner or Paramedic with experience across a range of health conditions, including acute, long-term frail and elderly, who will be able to support the on-going development of our Community based Team.
Interviews for this post will take place on 12th/13th September 2024.
Main duties of the job
The post holder will be confident in providing effective clinical leadership to the team and work well under pressure. They must possess advanced clinical assessment skills, be a confident prescriber, able to problem solve and think on their feet.
The successful candidate would also be required to lead and co-ordinate compassionate, high quality and evidence-based care by supporting the Clinical Lead and working in partnership with the AHP Leads and other ICT and Community Matrons, as well as system partners, including the two acute hospitals in the county, the ambulance provider and 111, to ensure the safe and effective delivery of the service throughout its operational times which may also include out of hours.
The candidate will require highly effective communication skills to ensure collaboration with the multi-disciplinary team, including the therapy service within the ICT function, and provide strong clinical leadership presence that promotes autonomy and development of the workforce.
You will need to provide strong leadership, possess excellent communication skills and be able to develop and support staff with clinical and professional expertise.
