Professional Nurse Advocate (PNA) and Personalised Care Lead
Hours/Days: 22.5 hours/3 days per week
Location: Remote working from home with countywide travel
We are looking for a unique individual to work in Northamptonshire Primary Care Training Hub (NPCTH) to support the nursing and personalised care workforce in Primary Care across Northamptonshire.
The Northamptonshire Primary Care Training Hub provides the infrastructure for multi-professional training and education in primary care to support the recruitment, retention and return of all staff groups.
The PNA and Personalised Care Lead will be a skilled specialist nursing advocacy role to support the nursing workforce working in Northamptonshire primary care to deliver safe and effective practice via the use of restorative practice supervision. The postholder will provide leadership and expert advice in relation to the PNA role. They will lead the development and implementation of a strategy to embed the PNA role across primary care in the county. They will work with qualified PNAs and PNAs in training to support, organise and coordinate activities to develop and embed the role and restorative frameworks into practice.
Main duties of the job
The PNA and Personalised Care Lead will also lead the development and subsequent implementation of a strategy to support the training and education of Social Prescribing Link Workers, Health & Wellbeing Coaches, Care Coordinators, Mental Health Support Workers and Mental Health Nurses working in primary care in the county. The role will promote and support these Personalised Care roles in general practice, aiming to increase both understanding and uptake of the roles in Northamptonshire. Personalised Care positions play a critical role in providing extra capacity and expertise to support patients to be actively involved in managing their health; to make choices that are right for them and reduce the demand on services.

- Educated to post graduate degree/Masters level or equivalent knowledge and experience.
- Registered Nurse or and have an active Registration with a Professional Regulator
- To be a qualified Professional Nurse advocate

- PNA experience in delivering the role within primary care setting

- Advanced interpersonal skills to manage challenging situations, which will include negotiation with placement areas in relation to capacity to support students
- IT literate with the ability to use electronic systems, diaries and databases, Microsoft packages, System1 and Microsoft Teams. Ability to deal with competing priorities on a daily
