Job summary

This an exciting opportunity to join the Community Specialist Palliative Care Team as a Team Lead . The Team Leam will be required to influence patient care by participating:

  • Leadership of the team and service development
  • High level of clinical practice
  • Coordination and triage of referrals into Hospice at Home
  • Training and education
  • Research and audit

The high level of clinical practice component will be achieved by responding to the physical, psychological, social and spiritual needs of patients and those who are important to the patient; ensuring the assessment and management of palliative care needs. . There is an expectation to ensure care reflects the strategic priorities of NHFT; ensuring a flexible, responsive service that meets the local needs of people across Northamptonshire, whilst upholding Care Quality Commission requirements

Main duties of the job

The band 6 post holder will be based at Cynthia Spencer Hospice, although there will be a requirement to travel across County; based on service need. This is a part time position of 22.5 hours.

The successful candidate must have significant experience as a Band 5 or above nurse, with a good level of knowledge and skills in Palliative Care; demonstrating true enthusiasm and energy for the speciality.

Previous experience in palliative care and leadership is essential for this post.

Excellent communication skills will be required and an ability to support people with complex emotional and psychological needs.

The successful candidate will work closely with the multi-disciplinary team in order to support the service, and have a high focus on delivering quality outcomes and improving the patient experience. You will receive clinical supervision, relevant training, peer support and appraisal to support you in your role.

Car owner / driver and current driving license essential or access to a car (unless you have a disability as defined by the Equality Act 2010).

Essential

  • Registered Nurse (Adult) /Current NMC Registration
  • Recognised qualification in palliative care, or working towards
  • Evidence of Continuous Professional Development
  • Demonstrate commitment to and recent evidence of further education and personal development
  • Completion of V300 Non-medical prescribing or willing to undertake within 2 years

Knowledge and Experience

  • Demonstrates significant and recent clinical knowledge in palliative care
  • Experience of leadership in clinical teams
  • Evidence of well-developed critical decision making skills which is evidence-based
