Job summary

The CATCH Team (Community Assessment and Treatment of Children at Home) are part of the Integrated Specialist Nursing team which comprises of the CATCH Team, Long Term Conditions, Training, Home Respite and the Nurse led Continence Service.

There are two main aspects of the service the CATCH Team offer:

CORE CATCH:

To care for children in the community with acute minor illnesses, providing an alternative to Hospital admission. The Team accepts referrals for children presenting with a variety of conditions such as wheeze, bronchiolitis and gastroenteritis. The service is open 8am-9pm 365 days a year providing short term support to children and their families/carers. This support is delivered by telephone consultations and home visits empowering families/carers and educating them on how to care for their child during this episode of acute minor illness.

CATCH 360:

Working in GP Hub settings as a first contact, assessing, diagnosing, and managing children under 6 years of age who are presenting with acute minor illness. Each clinic has a Duty GP that offers support to the team.

The post holder will be lone working in clinical practice, and we are looking to recruit a forward thinking, dynamic nurse into this permanent position. The post holder will be part of a developing and progressive service working with colleagues who present with enthusiasm for innovative and high standards of practice, embrace change whilst remaining flexible.

Main duties of the job

The CATCH Team provide a service from 8am-9pm, 7 days a week. The post holder will work as part of a team assessing acutely unwell children in their own homes/community, taking referrals from GPs, East Midlands Ambulance Service and the acute hospital. The aim is to reduce hospital admissions and manage the child safely within the community setting. There is an expectation that the post holder will cross work with the wider Children's Community Nursing Team, supporting children open to the services.

Each member of the team is part of an on telephone on call rota that is shared with the wider nursing teams under the Specialist Childrens Services. The post holder will gain skills to enable them to cross work with the wider Integrated Specialist Nursing Team & will take part in End of Life Care whilst being supported by wider team. The Trust offer Clinical and Safeguarding Supervisions on a group or individual basis. The Team is supported by a Team Lead and Clinical Lead.
