Job summary
Your key responsibility is to provide excellent clinical care & holistic support to patients and those affected by cancer at all stages of their cancer pathway from referral through to the process of investigation to treatment and beyond. You will facilitate & co-ordinate care across the wider multi-disciplinary team ensuring clear communication with patients, their relatives & other healthcare professionals.
You will be the person who is an accessible member of the MDT, taking a key role in coordinating the patient's care and promoting continuity, ensuring the patient knows who to access for information and advice. You be present at diagnosis and for treatment planning, acting as the patients advocate.
In your role you will promote and provide personalised care interventions for patients and those affected by cancer on your caseload.
Main duties of the job
Salisbury NHS Foundation Trust (SFT) are the local support team for head and neck, thyroid and skin services providing support, treatment and aftercare for those patients diagnosedwith a head and neckcancer. Following a cancer diagnosis, the patient's treatment is transferred to the Specialist Head and Neck Department in Southampton Hospital for eithersurgeryorneoadjuvant radiotherapy chemotherapy.
The Head andNeckCancerCNSteams atbothhospitals work together with the interdisciplinaryteam to help deliveran expert specialist service.
The Head and Neck CNS help to care for patients at all stages of their cancer journey in akind, compassionatemanner. Beingactive in clinical practice,multidisciplinary team meetings (MDT), management of patient caseloads, education, and audits. The CNS is presentat four of the multidisciplinary meetings, either locally (including skin)orin Southampton. Attending the clinic in Southampton on a Tuesdayafternoon.
