Our core mission is to provide patient-centred, intensive care and treatment to individuals and their families in their homes as an alternative to hospital admission. • Deliver intensive, short-term crisis interventions in line with the CRHTT clinical model.
  • Develop, implement and evaluate individualised care plans in collaboration with service users, carers and the MDT.
  • Assess and manage clinical risk, escalating concerns appropriately and taking assertive action to prevent harm.
  • Identify and respond to physical health needs as a routine and essential component of crisis assessment.
  • Undertake and/or coordinate physical health checks, including cardio-metabolic screening (e.g. blood pressure, BMI, smoking status, blood tests where competent).
  • Support engagement with primary care and specialist services, ensuring effective information sharing and follow-up.
