Are you an NMC qualified nurse seeking a new opportunity? We are seeking a full-time, dual role Physical & Mental Health Nurse to deliver integrated mental and physical healthcare to adults experiencing acute mental health crisis, with a particular focus on people living with Serious Mental Illness (SMI) who are at increased risk of physical health inequality.
You will provide senior clinical assessment, triage, and short-term intensive treatment within the Crisis Resolution and Home Treatment Team (CRHTT), while also ensuring that physical health needs are identified, assessed, monitored and acted upon as an integral part of crisis care.
Our core mission is to provide patient-centred, intensive care and treatment to individuals and their families in their homes as an alternative to hospital admission. Crisis care is a highly specialised intervention that involves supporting our most vulnerable service users in the least restrictive setting possible. This is achieved through recovery-focused, psychosocial interventions that help individuals regain or maintain independence.
The CRHTT (Crisis Resolution and Home Treatment Team) works closely with Inpatient wards, Inpatient social workers, Adult Mental Health Teams (AMHTs), patient flow transformation initiatives across inpatient and community services andoperates 24 hours a day, 7 days a week, 365 days a year, ensuring continuous support for those in crisis.
Main duties of the job
- Undertake timely triage, assessment and formulation of individuals presenting in mental health crisis.
- Deliver intensive, short-term crisis interventions in line with the CRHTT clinical model.
- Develop, implement and evaluate individualised care plans in collaboration with service users, carers and the MDT.
- Assess and manage clinical risk, escalating concerns appropriately and taking assertive action to prevent harm.
- Identify and respond to physical health needs as a routine and essential component of crisis assessment.
- Undertake and/or coordinate physical health checks, including cardio-metabolic screening (e.g. blood pressure, BMI, smoking status, blood tests where competent).
- Support engagement with primary care and specialist services, ensuring effective information sharing and follow-up.
