We're recruiting for a Clinical Lead Ward Liaison to join the Crisis Resolution and Home Treatment based in Aylesbury.

The Clinical Lead Ward Liaison role sits within the Crisis Resolution and Home Treatment (CRHTT) team but will be based for much of the time in the acute inpatient wards. The post holder will track inpatients through their admission, ensuring that when the purpose of the admission has been met, typically assessment, risk management and planned treatment/interventions, that the patient can be discharged back to the address from where they were admitted with home treatment support as necessary as soon as is safely possible.

Main duties of the job

  • To undertake autonomous assessments and risk assessments and support a positive risk-taking approach to the ongoing care pathway for patients whose needs no longer need to be managed in an inpatient setting.
  • To support the effective use and management of the inpatient resource, ensuring that capacity is maintained in the local inpatient wards to enable those patients with highest need to be supported close to home.
  • To become a member of the leadership teams in the adult acute wards, building relationships based on trust, openness and transparency.
  • To support the development of home treatment care plans that meet the immediate needs of patients and their carers/significant others at the point of early discharge from the inpatient wards using the entire resource that the CRHTT has on offer.
