Patient Flow Coordinator - Witney Hospital

Job summary

The Patient Flow Coordinator plays a pivotal role in ensuring the safe, timely, and efficient movement of patients through our community hospitals. This is an exciting new role which will require you to communicate effectively with patients, families and our multidisciplinary team in order to help patients get home as soon as they are ready. You will work with the teams at Witney Community Hospital where your work will directly contribute to reducing length of stay, preventing readmissions, and improving patient outcomes.

Main duties of the job

Key responsibilities of this role include:

  • planning and coordinating hospital discharges
  • effective communication with patients and families to involve them in preparing for discharge from hospital
  • working closely with colleagues in other organisations such as adult social care and voluntary sector organisations to arrange appropriate support for patients when they leave hospital
  • maintaining clear and accurate documentation
  • coordinating and contributing to daily board rounds and weekly MDT meetings
