Are you a qualified Social Worker seeking a new full time opportunity?
We are currently seeking a Mental Health Social Worker - North & West to join the Trust! You will be given an opportunity to work within the Adult Mental Health Team, providing social work expertise and statutory social care, ensuring that all service users with the AMHT have full assessment of their care and support needs and that support planning takes place to meet these needs.
This is an opportunity for a qualified Social Worker to join a multi-disciplinary team and make a difference to vulnerable adults and service users, whilst developing your career as part of a friendly team.
Please note, this role is a Band 6 post, but we are happy to look at applicants who could start the role as a Band 5 and develop in the post.
Main duties of the job

You will identify and address any barriers to promoting a social model of mental disorder, in partnership with service user, their family and support network and the multi-disciplinary AMHT

You will provide expertise with regards to application of the Mental Capacity Act and Mental Health Act.
- To participate in and contribute to the multi-disciplinary team, articulating the social perspective of mental health and ensuring that meeting the social care needs of service users retains a high priority and profile within the functioning of the team.
- To undertake Care Act or s.117 aftercare eligibility assessments some of which will be for people presenting with highly complex needs using your profession specific knowledge and skills.
- Intervene and show professional leadership and skill in situations characterised by high levels of social, family and interpersonal complexity and risk.
- To formulate social care support plans following the requirements of the Care Act 2014 and s.117 aftercare eligibility using the process required by the Local Authority.
- To identify and able to facilitate assessment of carer's needs and implement appropriate support plan
