Job summary
This is an exciting opportunity for an experienced and motivated ACP to join a supportive, multidisciplinary team dedicated to providing expert rehabilitation following stroke. You will:
- Deliver advanced assessment, diagnosis, treatment, and management for patients on the stroke rehabilitation pathway.
- Work autonomously, using advanced clinical decision-making to provide safe and effective care.
- Lead and contribute to service development initiatives, ensuring high standards of rehabilitation and recovery.
- Provide education, mentorship, and supervision to colleagues and students.
- Actively participate in research, audit, and quality improvement projects.
About youWe are looking for someone with:
- MSc in Advanced Clinical Practice (or working towards completion).
- Independent Prescribing qualification (or commitment to achieve).
- Significant clinical experience in stroke, neurology, or rehabilitation.
- Strong leadership, communication, and teaching skills.
- A passion for improving outcomes for stroke patients through innovation and evidence-based practice.

Main duties of the job
The Advanced Clinical Practitioner (ACP) - Stroke provides expert, autonomous clinical assessment, diagnosis, and management for patients across the stroke pathway, from hyper-acute care to rehabilitation.
Working as an integral member of the multidisciplinary team, the ACP delivers evidence-based interventions, supports complex decision-making, and ensures safe, timely, and person-centered care.
The role includes leading clinical practice, contributing to service development, and enhancing patient outcomes through advanced skills in assessment, prescribing, and care planning. The ACP will also provide specialist education, mentorship, and leadership, fostering continuous improvement in stroke care quality, patient experience, and team capability.
