Job summary

This is a part time Consultant post in child and adolescent psychiatry, specialising in eating disorders. The post is with the CAMHS eating disorders team based at Raglan House and serving the whole of Oxfordshire. If the applicant is interested in a full-time post, we would happily discuss how to develop this.

The post holder will work with the other consultant in the team (0.8WTE clinical sessions) and be supported by a speciality doctor who will work closely with the Consultants and 1 Consultant paediatrician who provides 2PA per week. Currently there is a further locum specialty doctor in the team. There is a full time ST5/6 in the team. The CAMHS ED team has a 0.8 team manager who is responsible to the Service Manager. There are arrangements in place for cover for annual leave, sick leave and study leave being provided within a group of consultants who cover ED, OSCA, crisis and LD.

Main duties of the job

The team currently assesses 2-3 new cases each week and the total team case load is around 150 cases. Referrals are received from GPs in Oxfordshire, school staff, social care, other CAMSH pathways and from self-referral. There are clear criteria for referral. Most of the cases are case managed by members of the multi-disciplinary team, involving the Consultant Psychiatrist as needed. The post holder would be able to take on a limited number of cases for family-based therapy and/or CBT-E, depending on their interest. The post holder would occasionally be required to provide second opinions for other teams and consultation and liaison with other teams.

Job description

Job responsibilities

To develop an active leadership role in the multi-disciplinary Child and Adolescent Mental Health Services (CAMHS), in conjunction with the teams manager and senior clinical staff. To provide diagnosis, management and treatment of children, adolescents and families from Oxfordshire who have an eating disorder (or suspected eating disorder) and require consultant psychiatric intervention. The consultant will carry medical responsibility only for those patients on his/her own caseload and that of medical staff under his/her supervision. As a guide, full-time consultants in the trust are expected to see 1 to 2 new cases per week, including an emergency slot each week, 10-12 follow up and approximately 5 non face to face contacts which might be school visits, social care meetings, phone appointments. This may vary week to week and is regularly reviewed in team case management and annual job planning. Within the eating disorder team, most cases will be care co-ordinated by non-medical staff, however, there will be some cases that are appropriately case managed by the Consultant. To provide clinical leadership within the multidisciplinary team and collaborate in systems of supervision and care management within the teams. The consultant will offer consultation, joint working or psychiatric assessments as appropriate for all patients from Oxon that are care coordinated by the eating disorder team. To work with Getting Help teams to further develop joint working and support the newly developed single point of access (SPA) service for all appropriate referrals to the CAMH eating disorder service. To work closely with Getting More Help CAMHS where patients have needs across both services. With other consultant psychiatrist colleagues to endeavour to assess all emergencies referred to them within the day of referral. When necessary, this will include responsibility for Mental Health Act assessment of patients. Urgent referrals are seen within a week and routine referrals within a month. To work in partnership with the multidisciplinary teams for the clinical management of patients in relation to the care programme approach (CPA). This will include risk management, the assessment of the needs of carers, and the development of services to meet the needs of patients and families for which the teams are responsible. To liaise with community and hospital paediatricians, adult mental health (particularly the adult eating disorder service) and other agencies dealing with the same population, such as education, social services and primary care where children or adolescents may be treated. To support and develop community mental health services for patients and their carers in Oxfordshire. The consultant psychiatrist will work with primary health care teams, other mental health practitioners, and other relevant agencies in the shared management of patients. He/she will be encouraged to develop services in line with the recommendations of the National Service Framework/NHS Plan modernisation programme. To share responsibility for the effective use of inpatient beds when necessary, ensuring prompt admission and discharge of patients from his/her catchment area. To provide reciprocal cross cover for consultant psychiatrist colleagues in CAMHS for annual leave, unplanned leave and for short term sickness etc, and in the normal working week to ensure that: Each consultant has protected time for Continuing Professional Development and other agreed activities. A consultant is available during office hours for psychiatric emergencies, Mental Health Act assessments, and other necessary consultation. Cross cover with consultant colleagues will be arranged on an equal basis and endorsed by the Clinical Director. To contribute to the Oxfordshire and Buckinghamshire CAMHS on-call consultant rota for child psychiatry. Senior mental health practitioners form the outreach service are first on call, with St4-6 doctors and speciality doctors providing the first medical response when needed in approximately 2 out of 3 nights. The on-call consultant rota for child psychiatry covering both Oxfordshire and Buckinghamshire is approximately 1 in 12 with prospective cover (category A, 3% + 0.33PA pro rata/week for intensity). The rota runs from 5pm 9am weekdays and all day for weekends and bank holidays. Duty cover is primarily telephone consultation. However, there is an expectation of occasional direct assessments in the community, police station or acute hospital settings, when needed, particularly if there is no junior doctor on the rota (approximately 1 in 3 nights) or if the complexity of the case is such that direct Consultant involvement is needed. In the weekday the multi-disciplinary colleagues see urgent appointments and run a duty system. All the community psychiatrists offer consultation for colleagues seeing emergencies. OOH psychiatric assessments in general hospitals, police stations and the Adolescent unit may be requested. OOH referrals are made through the Trusts Single Point of Access in each county. CAMHS OSCA Senior Mental Health Practitioner will take on first calls for CAMHS out of hours. On approximately 2 out of 3 nights junior doctors (ST4-6 and specialty doctors) outside of office hours take first line CAMHS responsibilities for referrals from the OSCA team. To participate in urgent and emergency clinical work undertaken by the whole multi-disciplinary team during normal working hours. The consultant provides senior medical cover, together with the other consultants for the sector/service. To consider as appropriate, involvement in special clinical interests: with the agreement of the Clinical Director and consultant colleagues within the locality, the post holder may develop a specialist clinical interest which contributes to the priorities of the service of which the consultant is part. Special interest areas are encouraged and have been agreed within our department and are reflected in the Oxfordshire CAMHs strategic documents. These may include specialist therapies such as family-based therapy, CBT-E, Adolescent-Focussed Treatment, parent group work or Multi-Family Therapy, or clinical ethics. Other areas of interest are encouraged. To participate in supervision in line with the Trust supervision policy. There is an effective consultant peer supervision group meeting prior to the monthly consultants meeting and other forms of clinical supervision are available within the team.
