Job summary

We are recruiting a Discharge Planning Coordinator to join our Patient Flow Community Hospital inpatient team. Hospital site to be discussed at interview.

This is a unique and exciting career opportunity for you to be part of our Community Hospitals team. The role is fast-paced and has evolved significantly during the COVID pandemic to support patient flow through the Community Hospitals and facilitate safe, smooth and timely discharges.

Main duties of the job

You will have a key role in our multi-disciplinary team and work collaboratively with your Discharge Coordinators colleagues across the Oxfordshire service

Ideally, you should have some experience of working within a healthcare setting and have a good understanding of community services. A willingness and commitment to learn, with compassion to delivering high-quality patient care is essential.

As part of the role, you will be supervised and supported by the Patient Flow leads and play a vital role in effectively communicating information to our patients, relatives, healthcare professionals and system-wide partners.

You will be planning each patients discharge from the first day of their admission to our services. Our staff are passionate about delivering the safest and best care which will enable and allow our patients to return home sooner after being admitted to hospital.

Our Oxfordshire Community Hospitals are based just outside Oxford in the surrounding beautiful towns of Abingdon, Bicester, Didcot, Headington, Wallingford and Witney. The wards care for male and female patients, providing in-patient rehabilitation, and specialist stroke rehabilitation.

There may be times where you will need to travel to other Community Hospital sites, therefore access to your own vehicle or access to alternative travel arrangements is essential.

Essential

  • Higher Education qualification (i.e. 'A' Level/NVQ III standard) or equivalent/previous experience in a Heath Care role
  • IT Skills to RSA3/NVQ Level 3, or equivalent experience
  • English to GCSE or equivalent

Desirable

  • Advanced ECDL or equivalent IT knowledge

Knowledge and Experience

Essential

  • Experience of working in a Health Care setting
  • Able to use a computer, with experience of using Office computer programs including Excel, Teams, Word and Outlook
  • Ability to liaise with all professionals and levels in a confident and effective manner
  • Ability to deal with the public in a polite and effective manner
  • Ability to manage and prioritise own workload
  • Ability to produce good quality correspondence and reports
  • Ability to minute meetings
  • Organisational skills/managing diary appointments and tracking information

Desirable

  • Working knowledge of NHS email systems, EMIS
  • Experience of working clinically in a health care setting
  • Proven experience in discharge planning in a Health Care setting
