Job summary

This is a unique opportunity for an Allied Health Professional (AHP) with knowledge of and interest in "needs-based rehabilitation" for chronic conditions. The ideal candidate will have both clinical and operational experience, be enthusiastic, willing to work hard, and deliver change and innovation. You will be prepared to work with the wider system and collaborate with social, health sector, and third sector organisations.

In addition, this post will support the transformation programme for the Pathway working with Head of Service, to provide Operational, Clinical and AHP leadership.

Main duties of the job

This post is a pivotal part of both the pathway's senior management team. Key to this role is ensuring the development of the community offer to support effective care delivery in the patient's home, community, clinics in line with the National and Community Services Directorate strategy.

The postholder is expected to contribute fully to effective partnership working with other organisations to improve the integrated health and social care offer to the local community. This role will contribute appropriately to Trust wide AHP and directorate wide tasks/ projects with the senior leadership team and corporate colleagues.

Key Responsibilities:

  • Lead and manage the professional, operational, and clinical service delivery of a nominated service area.
  • Develop a culture of quality improvement and empower nurses/AHPs to enhance care and well-being.
  • Ensure compliance with CQC domains: Safe, Effective, Caring, Responsive, and Well-led.
  • Collaborate with colleagues to address complaints and incidents, leading the learning from such events.
  • Provide strong managerial and clinical leadership, promoting an environment of creativity and innovation.
  • Manage the budget for the services and ensure financial plans are developed and implemented.
  • Lead the delivery of complex demand and capacity targets for the services.
  • Foster effective collaboration with patients, service users, carers, and other key stakeholders.
