The South AMHT is currently undertaking pilot projects in line with the Community Mental Health Framework in order to promote patient centred care and recovery. There will be changes to the framework in which we support patients and are currently moving away from Care Programme Approach.
The South AMHT consists of four satellite bases in Wallingford, Abingdon, Didcot and Wantage and consists of the multi-disciplinary team of Consultant Psychiatrists, junior doctors, psychiatric nurses, psychologists, support time and recovery workers, occupational therapists and the integrated social care team. We also work closely with embedded third sector services in each locality.
This is a Band 6 but will accept Band 5s for a Development Post.
Main duties of the job
The role will involve being the allocated key worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of an multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs.
We would like the successful candidate to undertake assessment of patients new to the team, offering biopsychosocial assessments and providing brief intervention as appropriate. We will also offer opportunity to work with patients longer term.Staff also undertake the duty role, which involves the triage of new referrals with the view to signpost people to appropriate services or offer assessments with the team in appropriate timeframes.You will need to be able to work autonomously, however there will be ongoing support from peers and the leadership team in the form of both informal and formal clinical and management supervision.You will also be able to critically assess and manage risk, including the use of Safeguarding processes.
