We are looking for a nurse experienced with working with 'Children We Care For' (Looked After Children) to join the Phoenix Team. The Phoenix Team comprises of specialist doctors, nurses and administrators who undertake health assessments and support the health management for Children We care For, children and young people at risk of / being exploited and Children and Young people subject to Court Orders under the Youth Justice Service.
The post holder should have proven experience of working with vulnerable children and young people in the community, be able to demonstrated the ability to work well within the team environment but also as an autonomous practitioner. The role requires an individual who is prepared to work flexibly, is able to travel across the county and work in a wide variety of settings.
As part of the wider Phoenix Team, you will undertake delegated review health assessments (RHAs) with the goal of addressing inequalities and improving health outcomes for children who are legally looked after by the Local Authority. It is anticipated that the majority of children seen will be under 5 years of age.
Main duties of the job
- To effectively engage with children and their carers by providing a creative and flexible service which aims to improve health outcomes in partnership with wider health and social care partners.
- To work in a flexible manner to ensure that health assessments are offered and undertaken for identified children within the 0-19 pathway. This will include working flexible hours and undertaking face to face and digital visits in a variety of settings across the county.
- Undertake high quality health assessments which are informed by health and social care information gathered prior to the assessment.
- Analyse the gathered information and assess, plan and implement packages of health interventions based on the needs identified during the meeting with the child and in partnership with other colleagues and professionals.
- Ensure that identified health recommendations are disseminated to the relevant professionals and concerns escalated as appropriate.
