Job summary
The Forensic Community Mental Health Service is expanding to include the Forensic Community Learning Disability Team. Therefore, we are seeking to recruit an enthusiastic, contemporary, and creative Social Worker who is passionate and caring supporting adults with intellectual development needs using evidence-based interventions and best practice guidance.
The post holder will work across Oxfordshire, Buckinghamshire and Berkshire and will have very close links with local Learning Disability teams and Local Authorities from each geographical area.
Main duties of the job
- Contribute to comprehensive assessments and transition of service users referred to FCLDT, both pre-acceptance to the FCLDT caseload and during discharge to local secondary services (LD/MH) or Primary Care/GP.
- Clarifying Local Authority responsible for Section 117 aftercare; where applicable identifying Nearest Relatives and appropriate family and carers involvement, determining previous input from LD/MH services and other relevant agencies and identifying any potential child protection or family issues.
- Link with Local Authority and Health Partners to ensure that Care Act 2014 and Section 117 Aftercare needs are identified and to ensure arrangements are made for effective discharge planning, and timely referrals for suitable accommodation are completed.
- Maintain contact with families and carers, where appropriate, and identify needs including referring for carers assessments when required.

- Develop a strength-based culture in practice and leadership within the FCLDT and with partner MDTs (such as hospitals during discharge and with consultancy work to other teams). Instil the social perspective during assessment, planning, and implementing person-centred programmes of care/interventions with teams and service users to give them greater choice and control over care they receive.
- Compile both non-statutory and statutory reports and present this orally for CPAs, Care (education) Treatment Reviews, Mental Health Tribunals, Hospital Managers Hearings and Conditional Discharge reports for the Ministry of Justice.
- Contribute towards the multi-disciplinary assessment and management of risk and being a lead on Multi-Agency Public Protection Arrangements (MAPPA) referrals. This includes taking account of peoples risk and safety at all times.
- Record, interpret and analyse information that can impact on risk and ensure the safety of others and the vulnerable adult.
- May act as a social supervisor for individuals that are conditionally discharged on S.41 if agreed with Local Authority.
