Job summary

We are looking to recruit a band 7 Senior care co-ordinator from a Nursing, Social work or Occupational Therapy background to join our established, supportive Older Adult Community Mental Health Team, based at our newly re-furbished South Bucks hub in Saffron House High Wycombe. This is a 12 months fixed term mat leave cover position.

We are looking for someone who can be part of a team, but also has the ability to work independently using their own initiative. The successful candidate would have excellent interpersonal skills, a strong understanding of Older Adults with Mental health needs, have an understanding of the CPA and be a strong advocate for training and development of junior staff and always considering the best interest of our patients. The team work with a large multicultural patient group of adults who are over 65, who experience severe and enduring mental illnesses.

Main duties of the job

The successful candidate will be a senior clinician within the team and lead a small team of newly qualified Nurses/Occupational Therapists, Nurse Associates, Peer Support Workers and Support Workers to provide care and interventions to a complex and large caseload of patients. The role be predominantly clinical, but include some operational leadership and supervision to your small team of junior staff.

You will work in collaboration with partnership agencies such as Bucks County Council, the Dementia Support services and Age UK, Private care homes and Substance misuse services. The majority of care will be in patients own home or their supported living accommodation.

We use the principles of the care programme approach and provide support for people in crisis and also those that need ongoing care co-ordination. We have incorporated patient centred care into the daily functioning of the team; which enables us to be focused on the recovery of the patient through a multidisciplinary (MDT) approach; with expertise from doctors, social workers, nurses, occupational therapists, support workers and psychological therapies staff. We also have twice weekly MDT meetings, where we discuss clinical matters within our supportive team. We strive to provide person centered evidence based care to patients, where possible working collaboratively with their families and carers.
