Job summary
We're looking for flexible, creative and skilled mental health clinician to take on the role of Patient Flow and Case Manager for Oxfordshire.
This role is crucial to ensuring our services run smoothly; you'll have oversight of all mental health inpatient admissions and the tasks required to manage a safe and effective discharge back to their own homes.
You will be responsible for minimising the number of out of area admissions but, where this has not been possible, to liaise with external providers in ensuring that the quality of care is of a high standard and that appropriate links are made with Oxfordshire community mental health teams to ensure safe and effective discharge.
If you have excellent communication skills and are able to work collaboratively with difference services, then this is the perfect role for you.
Main duties of the job
On a typical day you will review the local bed state, including health based place of safety (HBPOS) occupancy, complete required situational reports and support the clinical patient flow leads to undertake specific actions related to supporting flow on a given day. You will either chair the daily patient flow calls or support the clinical patient flow leads to chair and make decisions on bed allocations, considering priorities and risk. You will attend meetings with community and inpatient teams in addition to attending escalation calls and professionals meetings.
Throughout the course for the day you will be responsive to requests for use of HBPOS, new or updated bed requests and supporting with discharges from hospital where delays have been identified. At the end of the day you will again chair or support the chair with the afternoon patient flow call and then attend the final handover of the day to the duty manager and head on call. The role can be demanding yet very rewarding.
