If so, we have an exciting new role within the newly developed Autism & Learning Disabilities Liaison Team within CAMHS.

We are looking for an experienced and innovative Clinical Lead who will work collaboratively with key multi-agency partners, children/young people (CYP) and their families.

In this role, you'll be able to be system changer who supports CYP and their families to navigate the health and care system to achieve personal goals, enabling them to live more settled lives in the community and avoid unnecessary hospital admission.

You will need to be confident to identify and challenge barriers to success and support CYP and their families/carers to navigate, the often complex, health and care system that surrounds them.

Main duties of the job

You'll be supporting the Team Manager and Service Manager to develop this service within CAMHS Oxfordshire.

You'll be working in close partnership with multidisciplinary teams across health, social care and education, and with senior staff in partner organisations including Local Authorities, Thames Valley Provider Collaborative, Social Care, Community Agencies, NHSE Specialised Commissioning, Education settings, the voluntary sector and private provider organisations as required to develop joint working mechanisms and protocols to support the effective delivery of the ALDL service and the wider Dynamic Support Hub.

The team will utilise the Dynamic Support Register (DSR) and work closely within the Care Education and Treatment Review (CETR) process to identify the most at-risk CYP. The team will have oversight of their holistic programme of multi-disciplinary care and will proactively support the CYP to navigate, troubleshoot and respond to escalating needs through their range of interventions and pathways.

As Care Navigator, you will be the CYP's link person throughout their care journey until such time as their risk of admission decreases, facilitating a successful transition to alternative provision. The team will also escalate to appropriate pathways if the risk of the CYP increases.
