Job summary

We are looking for an enthusiastic, motivated, and driven Community Nurse to join our friendly, professional team in West Oxfordshire, based in Witney and Carterton.

The successful candidates will be providing high-quality, patient-centered care to individuals and families in their own homes and communities as part of a wider multi disciplinary team.

Main duties of the job

As a Community Nurse, you will visit housebound patients in their own home

You will conduct comprehensive assessments of patients' needs and developing individualised care plans. You will provide a range of nursing duties including wound care, medication management, chronic disease management and end of life care.

You will be liaising with other healthcare professionals in primary and acute health care and social services. You will support patients and families with self-care and disease management.
