Job summary
We are looking for Physical Health Lead Nurses to join our Older Adult Mental Health Teams.
One vacancy for North Bucks and one vacancy for South Bucks
The successful candidates can be either an RMN or an RGN andwill have excellent communication and organisational skills and be able to work well both autonomously and within a team using updated and evidence based physical assessment skills.
The candidate will be expected to champion Older People's issues regarding physical health in a variety of settings and professional groups, as well as to promote physical health care within the CMHT. You will work in partnership with the physical health leads in the wider Directorate.
Main duties of the job
The Older Adult Community Mental Health Teams are based in Aylesbury and High Wycombe in Bucks and we respond to referrals from primary care services and inpatient mental health wards, for older adults with severe and enduring mental illness. We would like someone to assist us with advice and guidance on physical health and to have robust physical health skills to provide assessment of older adults both on the hospital site and in their homes.
The team provides advanced clinical assessment, diagnosis and management of physical health, mental health and social care, using a holistic approach. The aim of the service is to keep people safe and independent for as long as possible in the community.
The leadership in this team is supportive, compassionate and transparent andthe needs of patients and staff are prioritised.
The successful candidate will be expected to use clinical reasoning and physical assessment skills to assess, diagnose and treat the physical and psycho-social needs of older adults with complex needs.
