We are looking for an experienced clinician to join us as a Patient Flow Lead (PFL) for our 6 Community Hospitals. You will work alongside another PFL and co-manage a team of Discharge Co-coordinators based in our rehabilitative Community Hospitals across Oxfordshire.
Main duties of the job
You must have excellent communication skills as you will act as a central point of contact for expert knowledge and information for our MDT, patients, their families and carers. Working closely with the MDT, acute, community and social care partners to ensure that barriers are removed and problems solved, allowing safe and timely discharge.
Whichever profession you are from you will work within your professional function, receiving appropriate supervision and using your unique skills to support both our patients and MDTs progressing our patients' journeys though to discharge.
You will have an up to date, working knowledge of current processes and awareness of the challenges faced in this area. An understanding of safeguarding, MCA and continuing care criteria is needed in addition to a range of clinical knowledge to support you in this varied role.
