Main duties of the job

The District Nursing service runs between 8am and 6.30pm, seven days a week. We can offer you a full time, part time or flexible working role within our South West Locality to include Wantage and Faringdon District Nursing teams. We work closely with patients registered with GP practices, liaising with a wide variety of community professionals and specialist services to provide high quality care in community settings.

We are a proactive District Nursing locality and work alongside GPs and other health and social care professionals to deliver a seamless patient pathway. Many of our patients are older and often have complex health needs. This role will be an opportunity to maintain and develop a wide range of clinical skills. To which you will be well supported to develop. We also offer an outstanding preceptorship programme for newly qualified or those new to the community setting.

We welcome both experienced community nurses, nurses who wish to move into Community Nursing or newly qualified nurses. We recognise that many nursing skills are transferable and that if you have a commitment to learn we will support you in your role.

The successful candidate will demonstrate excellent communication skills and will provide community nursing care, advice and information to patients and carers, with the aim of maintaining patients in their own homes and enabling them to enjoy maximum independence and an optimum quality of life. You will be supported closely with a robust induction programme.

Oxford Health Foundation Trust is part of the Buckinghamshire, Oxfordshire and Berkshire integrated care system who are one of seven national accelerator sites for the NHS Ageing Well initiative. This initiative is underpinned by the NHS long term plan with the aim of seeing patients in their homes within 2 hours or within 2 days.

Essential

  • Registered General Nurse (adult).
  • Educated to degree level or equivalent

Desirable

  • NMC Mentor qualification
  • Prescribing qualification.

Knowledge and Experience

Essential

  • Knowledge around long term conditions and their management.
  • Evidence of ongoing professional development.
  • Ability to supervise and support a range of individuals including team members, formal and informal carers.
  • Demonstrates leadership skills. Ability to plan and organise own and others work.
  • Demonstrates an understanding of local and national issues.

Desirable

  • Previous experience of implementation of change.
  • Previous Community Services experience.

Technical expertise/competence

Essential

  • Demonstrates an understanding of local and national issues influencing the delivery and development of health care.
  • Demonstrates a knowledge and understanding of evidence based care.
  • Demonstrates an understanding of the Clinical Governance requirements.

Desirable

  • European Computer Driving Licence or equivalent.
