Job summary

The post holder is responsible for actively participating as part of the Multi-disciplinary Team (MDT) within the ward/department, to provide a seamless service to patients.

To provide effective evidence-based care for patients referred to the service under the indirect supervision of qualified clinicians.

To implement, organise and deliver clinical programmes of care as agreed with supervising clinician in line with clinical governance and professional standards.

To use personal effectiveness and knowledge to contribute to the development and delivery of a high-quality, cost-effective service.

To ensure effective communication with patients, carers, and the referrers to the service.

The post holder will undertake the defined responsibilities appropriate to the role whilst complying with Trust policies, protocols, and guidelines they will ensure efficient and effective service delivery.

Main duties of the job

To monitor the patient delegated assessment responsibility, developing individual plans of care to meet the immediate and ongoing needs of the service user and carers .

Implement and progress patient interventions with autonomous assessing and planning and evaluation on a daily basis with indirect supervision of qualified practitioner.

Prioritise and effectively times manage clinically and other responsibilities. To implement, organise and prioritise the workload of junior member of staff.

To participate with the induction of staff new to the clinical area.

To co-ordinate activities around accurately document patient observations and early warning scores informing the registered nurse/midwife of any abnormalities.

Demonstrate clinical competency and high standards of nursing practise to other staff.

Coordinate and undertake the safe admission, transfer, and escort patients in accordance with bed management and Trust Policies. Perform effective and timely discharge planning.

Assist and support patients/clients in the activities of daily living.

To assist the registered nurse/midwife in the delivery of direct and indirect patient care ensuring that they are regularly updated on the patient's condition and progress throughout a span of duty.

Enable and support the patients in all areas of their activities of daily living.

Job description

Job responsibilities

To assist the registered nurse/midwife, by undertaking activities relevant to ensure the safe and timely discharge of patients.

Adhere to Trust Uniform policies.

To ensure the environment is clean and tidy.

To work independently.

To assist in the delivering of the Bowel Screening Programme.

o support qualified staff in the direct and indirect care of patients and other departments related to duties within all areas within the unit.

Monitoring patients observations during procedures and escalating and recognising any deteriorating in patients observations.

Maintain the patient mouth guard during upper endoscopy procedure once assessed and completed competence.

Support necessary patients positioning during procedures.

Prepare and support patients pre and post endoscopic procedure, following training and assessment you will be required to support the endoscopist/staff nurses with accessories during diagnostic and therapeutic procedures.

Prepare procedure rooms for endoscopic procedures ensuring that all necessary equipment is in good working order including scopes, endoscopic video equipment, diathermy plus argon, monitoring equipment and scope guide.

Ensuring safe checking and labelling of any specimens.

Ensure correct recording of any consumables items used during the procedure.

Carry out clinical intervention which is identified as part of the role and for which competency will be assessed, including ECGs Phlebotomy, cannulations, documenting and reporting results.

Use assessment tools to document pain score, sedation levels, pressure sores and nutrition.

To support staff nurses within the procedure room supervising sedated patients, performing routine observations, and ensuring safe recovery and assisting trained staff in taking of routine biopsies.

Accompany patients from and to other wards/departments as required.

Demonstrate basic cleaning and decontamination of equipment between patient use and storage.

To support within the decontamination of cleaning and storage of scopes.

Assist with the maintenance of department cleanliness as per daily and weekly cleaning list.

Assist with maintain stock level within the department.

Communicate effectively with all members of the endoscopy team and patients.

Take responsibility for own development learning and performance including participation in supervision.

Have a basic understanding of any long-term conditions that are diagnosed by endoscopy and the treatments and the principle of care and self-management.

Complete and document skills assessment for a wide range of therapeutic procedures that takes place during gastroscopy, colonoscopy, ERCP and Bronchoscopy in order to assist registered nurse.
