Job summary

Be an autonomous practitioner responsible for the triage and initial assessment, examination, initiation of treatments and investigations for suspected stroke patients adhering to protocols.

Implementing, co-ordinating, reviewing and evaluating care in collaboration with medical colleagues.

To be responsible for the co-ordination of assessment and appropriate clinical transfer of patients with a suspected stroke within the Emergency Corridor.

To be responsible for data collection to input to the Sentinel Stroke National Audit Programme (SSNAP), ensuring the patients receive best possible care in accordance to the Royal College of Physicians (RCP) Acute Stroke Management Guidelines.

Supporting staff in the Emergency department, Admissions Units & Stroke Units in caring for stroke patients.

Ensuring that accurate and timely information is given to the patient and/or their relatives.

Main duties of the job

Taking Pre-Alerts & referrals & triaging for priority.

Working alongside the stroke consultants & registrars to assess, treat & monitor hyper acute stroke patients in ED (including thrombolysis & mechanical thrombectomy.

Liaising with wider stroke MDT to ensure that patients have timely assessments.

Real time data collection for Sentinel Stroke National Audit programme (SSNAP)

Assisting with the monitoring of stroke service performance & participating in service improvement projects.

Supporting the nursing teams on the stroke units, teaching & coaching staff & students as the need arises.

Job responsibilities

Supporting Clinical Excellence

Acting as an advanced practitioner and professional role model, demonstrate advanced clinical skills, promote clinical excellence and ensure that an optimum standard of nursing care is delivered from admission to transfer/discharge.

Adhere to professional codes of conduct at all times.

Assist medical registrar/stroke SHO in assessing emergency patients, arranging for specific tests(blood, radiology etc.) and acting on them.

Triage and monitor all emergency patients with a suspected diagnosis of Stroke and act to ensure maximum standards of care are given.

Perform assessment and physical examination of emergency patients, initiate appropriate treatments and investigations.

Collect data to prove adherence to stroke protocols and performance against national stroke targets.

Request diagnostic investigations as appropriate, and act on the results of these.

Perform expanded practical clinical roles where appropriate cannulation, venepuncture, swallowing assessments, ECG recording and interpretation.

Ensure timely and appropriate transfer and/or discharge of patients from ward area, providing a seamless patient pathway.

Liaise with patents and relatives to ensure appropriate and informed care choices are enabled

Management Excellence

The effective management of all speciality specific patients through the emergency room. From assessment to admission and on to discharge.

To provide support to the Head of Nursing, Ward Sister and clinical leadership to the ward team.

Act as a professional role model and have an active role in clinical audit research and education.

Develop good communication networks to all members of MDT and liaise with areas across the trust, and develop networks at a national level

Ensure efficient bed management and seamless patient journey. Appropriately delegate work within the multidisciplinary team.

Deputise for Lead Stroke Nurse Specialist in their absence as required.

Assist in the management of staff providing mentoring and training support.

Education, Audit & Research

Demonstrate a commitment to evidence based practice, highlight areas for change, then initiate and evaluate evidence to improve and develop patient care.

As an advanced practitioner demonstrate expertise and share in depth specialist knowledge and skill to all team members both nursing and medical to develop clinical and professional nursing practice.

Provide active education and health promotion to patients and relatives as appropriate.

Identify and partake in clinical audit and research to improve knowledge and standards of patient care.

Provide a positive learning environment, participate in teaching and act as a clinical resource/role model. Liaise with internal and external educational and training providers to develop and motivate selfand other staff.

Act as an expert resource for the teaching of specialty care

Update advanced and essential skills on a regular basis to ensure that patients consistently receive a high standard of holistic care.

Maintain and develop professional portfolio to display competencies achieved and to display critical and reflective practice.
