Multiple Sclerosis Clinical Nurse Specialist -NeurologyBand 6 -

Full Time, Permanent 37.5 hoursper weekAn exciting opportunity has arisen for a Multiple Sclerosis Clinical Nurse Specialist, Band 6 to join our growing neurology team at Northampton General Hospital. You will be expected to work as part of our integrated, multi-disciplinary service and support the current team to deliver high quality care to people with MS as well as their families, throughout the disease trajectory, this will include newly diagnosed through to palliative care, complex needs, symptom management and the delivery and monitoring of all NICE approved therapies.

You will be expected to deliver care both within the hospital setting through outpatient nurse led and joint assessment clinics, inpatient reviews and within the community through outreach clinics and occasional home visits.

The applicant must have excellent communication skills and be dynamic, enthusiastic, motivated and able to proactively manage their own caseload, with team support, as well as part of a multi-disciplinary team to provide personalised health and social care.

You will receive training and professional supervision relevant to the role and support to develop specialist skills working with Multiple Sclerosis.

Main duties of the job

ClinicalAct as a specialist resource in MS nursing by promoting the service and increasing .awareness of long term, neurological conditions.

Ensure the quality of patient care is reviewed, assessed, implemented, and monitored to maintain standards of care given to patients and their families.

Participate in clinical audit of specialist nursing services to ensure evidence-based practice

Participate in the monitoring of standards of care within the defined policies, procedures and protocols of the service and directorate to ensure adherence to, and delivery of, a high-quality service.

Provide support and specialist advice to adult patients with Multiple Sclerosis, including information about the condition, treatment, and symptom management.

Assess patients clinical condition and following discussion with senior colleagues (medical and nursing) alter or initiate treatment of the patient ensuring a high degree of specificexpertise and care, which is in line with nationally agreed guidelines.

Provide a responsive telephone helpline for patients dealing with calls/queries and, following discussion and consultation with senior colleagues, for communicating plans to the patient/carer and all other members of the care team.

Responsible for improving and streamlining the process of care for patients through out their pathway in primary and secondary care.

Maintain accurate and contemporaneous clinical records in accordance with professional standards and Trust guidelines.

To assist in the management of the patient database.
