Job summary

The post holder will be responsible and accountable for the delivery of the aims and objectives of the Macmillan outreach project, scoping the information and support needs of the local community and developing a proposal for the delivery of cancer information and support.

To work with different groups to identify the holistic needs of people living with cancer and develop a comprehensive proposal for supporting individual communities at the end of the two-year project.

Main duties of the job

To develop effective working relationships with key stake holders, working with them to understand their cultural and individual preferences.

To work with the local community to identify innovative ways in which cancer information and support may be delivered.

Job responsibilities

To undertake a comprehensive baseline of the information and support available to those living with cancer identifying gaps in service deliver

To identify practical barriers to accessing information and support including travel, caring responsibilities, financial constraints, availability and the impact of health conditions

To map local support groups and their role in the delivery of information and support and link to the Macmillan website.

To identify the information and support needs of marginalised groups working with key stake holders about how these might be addressed

To establish how cancer information and support in being delivered in Primary Care, identifying the needs of those affected by cancer and healthcare professionals

To build effective relationships with key groups within the community working in collaboration to identify the needs of the local population

To explore how people living with cancer prefer to access information and support i.e., internet, phone, face to face, professional, peer support

To identify health promotion services and the campaigns to reduce the risk factors associated with cancer

To identify innovative ways in which cancer information and support may be delivered in consultation with the local population.

To engage with ethnic groups and other local communities to develop an understanding of the needs of the local population, exploring cultural barriers such as stigma around cancer, reluctance to talk about cancer and fatalistic beliefs.

To organise events for key stake holders to identify their informational needs, applying for a Macmillan grant to fund the event including cost of venue and any resources required. Ordering the resources required when funding received.

To explore the needs of the populations related to digital information and support, identifying any areas of exclusion.

To survey the needs of those living with cancer, exploring current information on offer and how beneficial it is in meeting the needs of cancer patients.

To develop a proposal for the delivery of cancer information and support as part of the development of the centre of excellence, identifying any resources required to meet the needs of those living with cancer.

To develop a Macmillan Investment Application for the delivery of cancer information and support, including comprehensive costings of the service delivery.

To scope the financial costing for the delivery of the cancer information and support model.

To develop and chair a steering group made up of patient representatives the local community and key stakeholders to identify the needs of the local population and develop a patient centred proposal for information and support.

To develop a project plan for the two-year program with agreed key milestones to be achieved in a timely manner.

To provide monthly reports to the Integrated Care System (ICS) through the Northamptonshire Cancer Board and Trust Cancer Strategy group.

To set up a patient/carer forum working collaboratively with those living with cancer.

To work with Macmillan Information & Support Lead and Macmillan Health and Wellbeing Coordinator to deliver information and support.

To explore ways to promote the Macmillan Information and Support Centre in different communities.

To work with voluntary and statutory organisations to scope information and support.

To scope the education and training needs of generalists providing cancer information and support.

To use both qualitative and quantitative method to deliver the project outcomes.

Review the results of the National Patient Experience survey to measure improvement in patient experience using the results of the 2020 as a baseline.

To review the results of the National Quality of Life survey to identify the unmet needs of those living with cancer.

Utilise feedback from local surveys/focus groups to inform the development of the proposal.

To audit the ethnicity and number of patients who access the HOPE program and other health and wellbeing events, obtaining feedback from participants.

To review the project plan and target dates on monthly basis via the steering group, identifying any delays in meeting agreed objectives, and making adjustments as required.

To work with a wide range of professionals, managers, volunteers, lay people and those living with cancer, demonstrating effective listening, communication and negotiation skills to identify the needs of patients/carers.
