Job summary
The main aspect of this role relates to providing an excellent level of care in the assessment and management of individuals referred to the Heart Failure Specialist Nurse service. A significant component of this work-stream comes through those individuals admitted to Kettering General Hospital with the condition of heart failure.
Main duties of the job
The post-holder will be informed of the various aetiologies underlying heart failure and the evidence-based management strategies employed to improve an individual's quality of life. As such, the post holder will be supported in reviewing patients referred to the Heart Failure team within both the acute setting, post-discharge and chronic disease management setting. In the former we will be offering a virtual ward which allows suitable patients to be managed at home with a combination of remote monitoring or home visits as required. Therefore, the post-holder must have a valid driver's license and access to a vehicle for work purpose.
In the latter two, this will include patient reviews within a clinic setting, delivering individualised care, facilitating lifestyle changes through education and support to patients, carers and other healthcare professionals. As such, the post-holder will work towards non-medical prescribing competency, with a view to initiation and up-titration of heart failure guideline directed medical therapies.
Our service also provides a direct assess telephone service which requires the post-holder to both prioritise and appropriately direct individualised care.
The post holder will work in collaboration with other members of the heart failure service, being supported by the Matron for Cardiology, Heart Failure Consultant Lead, Heart Failure Consultant Cardiologists and the HFSN Team.
