Job summary
We are looking for a motivated individual with excellent communication, interpersonal, and organisational skills, who is passionate about supporting patient care and working collaboratively as part of a busy and dynamic team.
Cancer Support Trackers play a vital role in ensuring patients are progressed through their cancer pathway smoothly and within national waiting time standards. You will be responsible for monitoring and updating patient pathways from initial referral through diagnosis to treatment, working closely with Multi-Disciplinary Team (MDT) Coordinators, clinicians, and the wider Cancer Services team.
At any given time, the Trust is responsible for tracking hundreds of cancer patient pathways. This requires strong attention to detail, excellent time management, and effective communication to coordinate diagnostic tests, procedures, and admissions both within the Trust and with tertiary centres.
You will use a variety of IT systems to maintain accurate patient records, identify potential delays, and ensure all information is updated in real-time. Supporting the team to meet national cancer targets, you will contribute to the delivery of safe, efficient, and patient centered care.
Main duties of the job

Accurately locate patient information from a range of systems to support the recording of all events along a patient's journey, highlighting any delays to the relevant MDT Co-ordinator

Proactively track and monitor patients along their cancer diagnostic and treatment pathways, ensuring timely progression in line with national Cancer Waiting Time standards.

Accurately update and maintain patient records across a range of hospital systems, identifying and escalating any delays or pathway issues to the relevant MDT Coordinator.

Liaise with clinical teams, departments, and other Trusts to obtain information, confirm diagnostic plans, and help expedite appointments or investigations where required.

Support MDT Coordinators with preparation and follow-up for weekly Multi-Disciplinary Team meetings, including sourcing and returning patient notes and ensuring accurate record keeping.

Review and action correspondence received via the generic NHS.net mailbox, logging upgrades and ensuring all cancer patients are correctly tracked on the database.

Generate and check reports as required to support accurate data capture and performance monitoring.

Maintain the highest standards of data quality and confidentiality at all times.
