Job summary

Kettering General Hospital Critical Care Outreach team is an integral part of the Critical Care team, providing a comprehensive service across the hospital site. This is an opportunity for an enthusiastic, highly skilled nurse to join our existing established team on a 12 month fixed term contract to cover maternity leave to provide high quality and professional services for the 'at risk' patient across all specialities.

You will contribute to the smooth running of the CCORT, offering site wide support with the aim of assisting in the early recognition and management of acutely ill deteriorating patients.

You will be required to supervise the assessment and interventions for the acutely ill patient in hospital supporting wards in the recognition and response to acute illness in adults. Through audit you will ensure that high standards of patient centred care are maintained whilst acknowledging the cost effective use of resources. You will practice at an advanced level demonstrating a high level of autonomy and clinical decision-making.

You will need exceptional interpersonal, communication and decision making skills, along with stamina and flexibility in order to meet the demands of the role.

As the CCORT provide a 24-hour service, the successful applicant must be able to work both night and day shifts.

Main duties of the job

CCORT post holder will be a clinically credible nurse or allied health professional with the expertise and specialist knowledge to deliver high quality evidence based care to the deteriorating ward based patient.

They will act as a role model providing support, education and training to the multi-disciplinary team caring for the deteriorating patient across Kettering Hospital.

The post holder will provide 24-hour outreach cover to primarily provide advice and support for acute deteriorating patients in the Trust environment.

Working both night and day shifts, the successful applicant will be involved in the early detection and management of acutely ill patients, assist in timely admission to the Critical Care area and involve the appropriate members of the MDT in the decision-making process when determining escalation plans.

Discharges from Critical Care are also followed up to ensure smooth transition from level 2 to level 1 environment.

Assisting with level 2/3 transfers within the organisation when required. The post holder will, where possible, attend 2222 calls in all areas across the trust. Within this role the post holder will provide expert advice, advanced skills and support to the Cardiac Arrest/ Emergency teams.

This role will require the individual to take responsibility for leadership where required to ensure patient safety and compliance with national guidelines.
