Street Triage Clinician to work within our Multi Agency CNTW/Northumbria Police Street Triage Team

1x Full Time Permanent Position

The STT operates with an experienced Band 6 Mental Health (MH) clinician and a Police Officer between the hours of 10am and 3am, 7 days a week (including Bank Holidays), this being the peak time for referrals and intervention.

Referrals for people of any age are received via the Police control room where the person is not already detained in police custody and there are concerns in relation to the person's mental health.

A triage assessment will be undertaken with the individual so that any risks can be reviewed, a safety plan formulated, and any appropriate onward referrals made. Where clinically indicated, the team will always endeavour to prioritise a face-to-face triage. However, the team also offer information shares and advice to police colleagues and telephone triages.

It is the responsibility of the MH clinician to signpost or make necessary onward referrals to services in the community when this is indicated. If following triage, it is evident that the individual requires detention under s136, or further community assessmentthe Street Triage Team will discuss this with the officers at the scene and the appropriate arrangements will be made.

Main duties of the job

o Clinician will work alongside STT Officers to provide timely advice and intervention including risk assessment, mental state examination to officers at the scene with individuals requiring support.

o Clinician will access electronic health records in order to obtain relevant risk information.

o In collaboration with officers at scene, the clinician will ensure all current risks have been discussed and explored with the individual in order to determine the most appropriate response.

o The MH Clinician will be responsible for all aspects of clinical decision making.

o The clinician will clinically triage calls and will make a collaborative decision on the most appropriate intervention.

o For face to face assessments, the clinician and STT officer will attend the scene and the clinician will take the lead in this intervention.

o The MH clinician will formulate a plan with the person including safety planning and agree any necessary follow up.

o The clinician will act as the service user's advocate.

o The MH clinician will act as an advocate for carers and families, including offering appropriate levels of support and providing information or signposting to other services, where indicated.

o It is the responsibility of the clinician to complete all relevant documentation in the individual's electronic health record following any contact, including onward referrals, signposting or updating care teams.

Qualifications

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post
  • Proven track record of post registration clinical experience
  • Mentor Training.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Knowledge

Essential

  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
  • Experience of working with older adults

Desirable

  • Understanding of difficult to manage/challenging behaviours.
