Job summary

Memory and Later life services are available throughout Cumbria. They offer assessment and a range of evidence based interventions in a variety of settings including the person's home, GP surgery, Care Home, clinic or hospital, depending upon the individuals' circumstances.

The service is for anyone who has new or suspected memory problems, including those with a diagnosis of dementia, and people in later life who have anxiety, depression or psychosis made more complex by physical health needs.

The teams work closely with the person and their family, as well as colleagues in other parts of healthcare, Adult Social Care and other community and third sector services. Staff within the service include Consultant Psychiatrists, Nurses, Occupational Therapists, Psychologists, Assistant Practitioners and Support Workers.

North Cumbria Memory and Later life services - EAST are looking for a Full Time Community Practitioner.

We are looking to recruit motivated and committed individuals into the team, with experience of working with Older Adults.

Main duties of the job

To be responsible for the review and management of a caseload of patients who have substantial and complex needs.

Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.

You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision

Work collaboratively with patients to develop care plans, recovery and wellbeingplans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary.

To consider carer needs and ensure carer and family are involved in care planning where appropriate.

To deliver evidenced based clinical interventions.

The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Coordinate treatment planning, interventions, clinical reviews and discharge planning.

To provide compassionate care that is based on empathy, kindness, respect and dignity.

To incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.

Education and Qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post and in Older Adults pathways
  • Proven track record of post registration clinical experience
  • Mentor Training.
  • Experience of trauma informed care, Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Post Registration Qualifications in Psychological Interventions
  • Experience of working within a community team.
  • Non-Medical Prescribing

  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
