Job summary
This is an exciting opportunity for you to work with the CNTW pharmacy team. You will work between our Central (Newcastle) and South (Sunderland) teams to have a role in direct patient care with opportunities to develop your clinical knowledge and skills.
We welcome applications from registered Pharmacy Technician colleagues or those Pre-Registration Trainee Pharmacy Technicians who are due to qualify before April 2026 working in all sectors of the profession.
We provide pharmacy and medicines optimisation services to all of our mental health, learning disability and neuro-rehabilitation inpatient areas, as well as to community and crisis teams. Our Pharmacy staff are highly regarded as valued members of the multi-disciplinary team.
Main duties of the job
The main duties of the role will be:
o Medicines reconciliation and review on admission.
o Providing support and information to patients and carers to assist with the management of their medicines.
o Optimising medicines, taking a holistic view of both mental and physical healthcare needs.
o Providing physical health and wellbeing advice to patients, carers and staff.
o Discharge planning, liaison with patients, carers, community teams and pharmacies.
Operational support to the main dispensary at SNH by performing regular accuracy checking slots.
