Job summary
Memory and later life service is for anyone who has new or suspected memory problems, including those with a diagnosis of dementia, and people in later life who have anxiety, depression or psychosis made more complex by physical health needs.
The post is for vacancy as a band 5 community nurse in our service.
Our team work closely with the person and their family, as well as colleagues in other parts of healthcare, Adult Social Care and other community and third sector services. Staff within the service include Consultant Psychiatrists, Nurses, Occupational Therapists, Psychologists, Assistant Practitioners and Support Workers. We are a supportive team with a variety of experience.
We offer assessment and a range of evidence based interventions in a variety of settings including the person's home, GP surgery, Care Home, clinic or hospital, depending upon the individuals' circumstances.
Main duties of the job
To be responsible for the review and management of a caseload of patients who have substantial and complex needs.
Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.
You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision
Work collaboratively with patients to develop care plans, recovery and wellbeingplans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary.
To consider carer needs and ensure carer and family are involved in care planning where appropriate.
To deliver evidenced based clinical interventions.
The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
Coordinate treatment planning, interventions, clinical reviews and discharge planning.
To provide compassionate care that is based on empathy, kindness, respect and dignity.
To incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.
