South Tyneside Older Adult's Community Treatment Team
We are seeking to recruit a skilled and motivated community practitioner to join our experienced and effective team.
The successful applicant will have an interest in working with older adults and have appropriate knowledge and skills to support this client group. We are looking for someone who is passionate about working with older adults who can work effectively as part of a team whilst also being able to work autonomously in this demanding role.
Please note this role will be based in South Tyneside Locality,Team base is Monkwearmouth Hospital.
Main duties of the job
As a community practitioner you will provide evidence based interventions, risk assessment and management to a broad range of people with functional and organic mental health needs. You will be responsible for the assessment, planning, implementation and evaluation of care needs and be responsible for the clinical management of a defined caseload. You will be supported within this role by an approachable and experienced leadership team who are committed to clinical supervision and professional development.
You must strive to deliver high quality, safe patient care and be a good role model in reflecting the trusts core values whilst offering leadership and supervision to junior members of staff.
There will be opportunity to work within the different functions of the team such as care home liaison, step up, diagnostic pathway as well as augment with other services to support patient care.
The team serves the population of South Tyneside and is an established team with a friendly and supportive culture.
Our current working hours are 9am-5pm Monday to Friday (excluding bank holidays) with occasional need to work out of hours to meet individual patient/service need, between 8am and 8pm.

Qualifications
Essential
- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
- Mentor Training.
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
Desirable
- Experience working with older adults
Knowledge and Experience
Essential
- 12 to 24 month band 5 qualified nurse experience dependent upon service need.
- Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
Desirable
- Understanding of difficult to manage/challenging behaviours.

- Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
