Job summary
We are excited to offer a full-time Band 5 Community Nurse post within the Gateshead Community Treatment Team in the West of Gateshead. The postholder must obtain a nursing and midwifery council registration and hold a nursing degree or qualification equivalent.

As a Community Nurse, you will have a genuine interest in collaborative working with adults with severe and enduring mental health needs.
As a registered professional under the NMC, this is an ideal opportunity for someone who wants to develop their community clinical skills.
As a core member of the team, you will also be responsible for your own caseload, providing ongoing assessment, safety planning, agreeing collaborative care plans as a Key Worker, and delivering evidence based intervention in the community.
Part of your role will involve working within the teams depot clinic & clozapine clinic, which provides medication management, monitoring and administration.
Main duties of the job
- Provide direct care and therapeutic support to individuals with mental health conditions.
- Creating collaborative care plans with service users.
- Arranging review appointments in keeping with service specification and the service user's point in their care pathway.
- In collaboration with service users, reviewing their mental and physical health, wellbeing and progress towards goals.
- Producing written records of reviews in the electronic patient record and completing all relevant paperwork associated with reviews, including letters.
- Work as part of a multidisciplinary team, coordinating with doctors, social workers, psychologists, and other healthcare providers.
- Complete referral's to other services for further ongoing support where needed.
- Using clinical supervision effectively.
- Conducting risk assessments andde-escalating crisis situations.
- Building therapeutic relationships with patients.
- Collaborating with families and other healthcare professionals.
- Administering and handling of depot injection medications.
- Promote recovery.
- Have awareness of the mental health act & support patient by providing in reach support to wards.
