Job summary
We are looking for a permanent, full time Community Practitioner to join the East memory and later life service in North Cumbria.
We would love to hear from you if you are interested in working with older adults offering assessment of their cognitive functioning.
The team is well established with approximately 50 well motivated employees working hard to assess, diagnose and treat those referred to our service.
Main duties of the job
To be responsible for the assessment and management of a caseload of patients who have substantial and complex needs.
Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.
You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision
Work collaboratively with patients to develop care plans, recovery and wellbeingplans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary.
To consider carer needs and ensure carer and family are involved in care planning where appropriate.
To deliver evidenced based clinical interventions.
The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
Coordinate treatment planning, interventions, clinical reviews and discharge planning.
Provide clinical supervision to staff as delegated by Team Manager and Clinical Leads.
To provide compassionate care that is based on empathy, kindness, respect and dignity.
To incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.

- Proven track record of post registration clinical experience relevant to the post.
