This is an exciting job opportunity for Community Practitioners within Newcastle and Gateshead CYPS within the Neurodevelopmental Assessment Team.

We would like to hear from practitioners interested and experience in community work and working with children and young people.

Main duties of the job

We are looking for practitioners who have experience in or transferable skills to manage complex caseloads, effective autonomous diary management and excellent written communication skills. The role will comprise of holistic assessments, carrying a clinical caseload and duty work.

To be responsible for the Assessment and Management of casework of service users who have substantial and complex needs and should be confident in clinical decision making.Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.In collaboration with the Service User and parents/carers, gathering detailed information.To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriateThe post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting wellbeing and maximising independence.Coordinate assessments as Care Coordinator within an MDT 'pod' from initial assessment, to follow up and discharge.

The post holder will write comprehensive diagnostic reports which are completed in a timely manner and to a high standard.

The service operates within the Central business unit of CNTW and works across the geographical area of Newcastle and Gateshead.

The team functions as an MDT where we work collaboratively with nurses, support workers, occupational therapists, social workers, psychology and psychiatry.

Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.

Demonstrate flexibility in the role. This may involve working across clinical pathways to support the needs of the service, whilst ensuring the delivery of high quality care at all times.

Education and Qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Post registration qualifications in psychological interventions
