Job summary

An exciting opportunity has arisen for a Band 5 registered nurse, who has completed their preceptorship, to work within the North Tyneside Primary Care Physical Health Team.

Your role will be supporting GP practices across 4 Primary Care Network's in North Tyneside in undertaking annual mental and physical health reviews for patients with a diagnosis of severe mental illness (SMI); such as Psychosis and Bipolar Disorder.

The North Tyneside Primary Care Physical Health Team is aimed at reducing premature mortality and enhancing the quality of life for people living with Severe Mental Illness (SMI). The Service promotes parity of esteem for patients presenting with mental and physical health needs.

The service aims to increase the number of people with SMI completing the full range of Physical Health checks and supporting better health and wellbeing. The service enables and empowers individuals by fully involving them in every aspect of their care and treatment. Patients will be supported in the development of their own physical health and wellbeing plans, crisis and support plans.

Main duties of the job

The successful candidate will be responsible for carrying out annual health checks; including Blood pressure, weight and phlebotomy.

They will review mental health presentations and make appropriate referrals depending on need.

They will work in collaboration with the patient to produce an enhanced wellbeing plan; which incorporates both physical and mental health interventions.

They will signpost and/or refer to other services including primary, secondary and third sector and improve patients knowledge of services in their area.

They will provide brief interventions in smoking, weight management and any substance/alcohol use where appropriate.

They will provide follow up for any referrals/signposting made in a timely manner.

The successful candidate will be expected to be able to meet the geographical requirements of the role.

  • First Level Registration RMN/RNMH/RNLD Qualification
  • Experience of working with service users and carers in successful delivery of treatment plans
  • Complete preceptorship programme for newly qualified nurses
  • Multi-disciplinary team work.
  • Proven ability to develop therapeutic partnership with service users and carers.
  • Proven ability to be able to communicate interventions to service users.

Desirable

  • Physical Health assessment skills
  • Further relevant post qualifying training
  • Previous experience in a community setting.
  • Experience of involving service users/ carers in evaluation/planning and monitoring of services

Knowledge and Experience

  • Experience during training or post qualification working in a mental health setting
  • 12 months post registration experience and completed preceptorship
