If this is you then the Northumberland Early Intervention in Psychosis (EIP) Team would like you to join us. We are currently offering a 9 month fixed term Band 6 Community Practitioner post.
You will be working across Northumberland supporting 14 - 65 year olds who are thought to be developing a first episode of psychosis.
You will work as part of a dynamic Multi Disciplinary Team (MDT), assessing and delivering interventions to service users as well as supporting families, whilst also working collaboratively with other professionals.
The service is committed to developing the skills of practitioners, so there will be opportunities to develop further clinical skills. You will be involved in care co-ordination, but with a caseload of 15 allowing greater opportunity to offer interventions / treatment.
Main duties of the job
Do you have a passion to support service users in their recovery, engage and involve families in care planning and use formulation to help a service user understand their experiences?
In the role as a community practitioner you will:
- triage referrals and allocate for assessment where appropriate.
- undertake initial assessment, feedback to MDT.
- act as point of contact for referrals and calls where they are not able to wait.
- manage a caseload, complete risk assessment and care planning, liaise with other professionals within and external to the team .
- provide recovery focused, evidence based interventions and care planning.

Education/ Qualification
Essential
- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
- Evidence of post-registration training/awareness in delivering psychological interventions.
- Mentor Training.
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
- Up-to-date knowledge/experience of relevant legislation.
Desirable
- Experience of working within the community.
- Post Registration Qualifications in CBT/Family Interventions
Knowledge and Experience

- 12 to 24 month band 5 qualified nurse experience dependent upon service need.
- Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
