The Clinical Risk and Investigation Department are looking for a Dynamic, motivated and passionate candidate to join the team as one of our investigating Officers. The purpose of the role is to investigate any serious incidents identified within Cumbria Northumberland, Tyne and Wear NHS Foundation Trust.

You will be required to work with clinicians and managers to identify significant risk across both clinical and non-clinical areas. You will ensure families and or Service users are involved and supported in the process so that they are part of the learning review where ever possible, taking into consideration consent to share complexities.

Main duties of the job

The post holder will ensure that the family's needs are met with regard to the provision of timely and accurate information during the investigation, openness and transparency and to reflect the level of involvement they wish to have in the investigation.

You will arrange and chair after action review meetings with all significant and relevant parties as well as utilise other forums to review an incident.

You will be required to produce and deliver both After Action reviews (AAR's) and Patient Safety Incident Investigation Report (PSII's). These will be presented to senior level staff through safety Group meetings and the Patient Safety Learning and Improvement Panel, which requires accuracy and the ability to communicate highly complex and sensitive information.

The role requires the highest degree of sensitivity, communication and integrity. You should have experience of collaborative working across statutory and non-statutory agencies and have experience in a senior clinical and or managerial role. You will require an understanding of the key Patient Safety Incident Response Framework (PSIRF).

Education and Qualification

Essential

  • To be educated to Masters level or a relevant clinical / professional qualification.
  • Five years post qualification experience.
  • Excellent management / leadership skills.
  • Evidence of continuing professional or managerial development.

Desirable

  • Teaching qualification
  • Root Cause Analysis training
  • Human Factors training

Knowledge and Experience

Essential

  • Interpretation and implementation of national strategies into local initiatives.
  • Ability to provide specialised advice to clinical and non-clinical areas on service development and re-design initiatives in line with the Trust's business
  • A good working knowledge of the requirements of clinical service delivery in Inpatient, Community and Specialist services.
  • This will include the performance management agenda in the NHS and clinical governance, and the needs of service users across the Trust.
  • Good understanding and working knowledge of the Serious Incident Framework (March 2015)
